Required forms for applicants with an Out-of-State driver's license:

• Driver Safety Program Registration Form
• MVR Authorization Form
• FCRA Disclosure Form

Email your completed forms and a copy of your driver's license to Robin Walinski or bring to the Office of Risk Management located in Fountain Hall, Room 114.
