Incident Reporting

In addition to mandatory reporting procedures, adverse incidents involving minors should be reported to the Office of Risk Management using the Youth Program Incident Report.  This report should be submitted within 24 hours of the incident occurring.  Risk Management will follow up with the submitter if additional support or information is needed.

Types of Incidents to Report

  • Any medical incidents that require more than basic first aid or result in a minor needing to be seen by a medical professional for treatment.
  • Conduct violations that result in the suspension or dismissal of a minor or program staff from a program including:
    • Consumption, use, or possession of alcohol or illegal drugs.
    • Abuse or distribution of prescription drugs.
    • Possession of legal items with the intent to be used for intoxication or impairment.
    • Possession or use of weapons, explosives, or fireworks.
    • Physical altercations or bullying of any kind.
  • Discriminatory behavior or harassment based on race, color, age, religion, sex, sexual orientation, gender identity or expression, national origin, veteran status, disability, or genetic information. 
  • Damage to or theft of University property
  • Other incidents that may arise and warrant reporting to Risk Management.

This list is not inclusive of all incidents. When in doubt, report the incident.
